Healthy Start is looking for a Bi-Lingual (fluent in English/Spanish) person to provide Care Coordination services to our participants in Jackson and surrounding counties. Healthy Start is a home visiting program that provides services to prenatal and infants. Candidate must feel comfortable completing home visits, documentation and following deadlines.
PRIMARY RESPONSIBILITIES
Home visitation services
Be able to meet with participant in home or office setting
Document using the Healthy Start data base system
Perform general duties to include documentation, scanning, faxing, and filing.
Maintain hard copy and electronic filing system.
Participate in and complete all trainings necessary to job description
Be prepared for regularly scheduled supervision with Program Manager
Complete and participate in meetings and conferences.
Maintain weekly schedule.
Support other staff members in tasks as needed

KNOWLEDGE AND SKILL REQUIREMENTS
Prefer experience in the area of home visitation with a degree, but will train candidate with qualifications that meet the standard of Para-Professional.
Duties require professional verbal and written communication skills. The ability to meet and talk with all demographics. The ability to maintain professionalism.
Salary will be negotiable, based on experience and education
